LITITZ, PA — Landis Homes has earned national recognition for excellence in post-acute care, ranking tied for #9 in the United States and #1 in Pennsylvania for the lowest hospital readmission rates among skilled nursing facilities (SNFs). The data, released by the Centers for Medicare & Medicaid Services (CMS) and published in Becker’s Hospital Review on March 26, highlights Landis Homes commitment to resident health and safety.

The report analyzed risk-standardized rates of potentially preventable hospital readmissions within 30 days of discharge, during the period of October 1, 2021, through September 30, 2023. The national average readmission rate was 10.51%, while Landis Homes achieved a significantly lower rate of 5.25%, reflecting nearly half the national average. In addition to this ranking, Landis Homes holds a 5-star overall rating from CMS, further underscoring the consistent quality of care provided to residents and their families.

Hospital readmission ratios are widely regarded as one of the most important quality indicators for skilled nursing facilities. They measure how effectively a facility supports recovery and prevents complications that might otherwise result in a return to the hospital. A lower readmission rate is generally associated with better care coordination, fewer medication errors, and more attentive monitoring of residents’ health conditions.

Beyond clinical outcomes, readmission rates carry significant financial and reputational weight. Under Medicare’s Value-Based Purchasing (VBP) Program, SNFs are rewarded or penalized based on performance in this area. Additionally, CMS publicly reports readmission data, which can affect a facility’s star rating and public perception. Strong performance, like that demonstrated by Landis Homes, signals a high-functioning care environment with strong systems in place to support residents and their families.
